The membership billing period starts on the date of purchase, and you will be charged monthly. The initial membership period shall be for a period of six months (the "Initial Period"). After the Initial Period, the membership shall automatically renew monthly. There are no cancellation fees if cancelled after the initial period. If the contract is cancelled before the 6-month commitment, there is an early cancellation fee of $200 that will be charged to the card on file. Either party may cancel the membership at any time by giving a 30-day written notice to the other party.
At any point within your contract, you may upgrade to a higher tier, however, you may not downgrade while you are in the "initial period." If you would like to downgrade your membership after your initial 6-month period, you may do so at any time by giving a 30-day written notice to Mediluxe.
Membership payments are non-refundable, and its perks are non-transferable and cannot be assign to another party. Memberships cannot be combined with any other offers or promotions. Unused services reset and do not roll over to the next month.
Any membership fee that is returned for insufficient funds, incorrect card number or card information is considered in arrears and due to Mediluxe. If a membership is considered in arrears, membership services and discounts will not be available for use until balance is paid. The member is responsible to ensure the credit card number on file with Mediluxe is valid and current. If there is not a valid credit card on file at the time the monthly fee is charged suspension of the membership will occur until payment is made in full.
Mediluxe reserves the right to review subscriptions periodically. Members will be given at least a 30 days' notice in writing of any changes, which may include: (i) any increase or decrease in membership fee, or (ii) change in date of automatic withdrawal.
Scheduling Policy:
A minimum 24-hour notice is required to cancel or reschedule an appointment. If such notice is provided, your scheduled complimentary treatments will not be forfeited. Failure to provide 24-hour notice or not showing up to your appointment, will result in forfeit of that monthly service. Mediluxe does not provide service rollovers, services reset and do not roll over to the next month. For booked, discounted services that are part of your membership benefits, appointments not canceled with 24 hours are subject to the general cancellation policy.
It is recommended you schedule your next month's appointment while in the office. If you fail to schedule your monthly appointment, you will lose out on your membership service for that month and still be charged your monthly fee.
Cancellation or Suspension of Membership Policy:
Memberships require at least a 6-month commitment. Once the commitment is made, no cancellation fees apply. If cancelled prior to commitment, the member will be charged a $200 cancellation fee. Members can cancel their membership at any time by giving a 30-day written notice to Mediluxe.
Mediluxe reserves the right at any time to cancel or suspend the membership of any member in the event of the following:
- The member commits a serious breach of this Agreement and/or Mediluxe Rules and Regulations.
- Where any monies are due to Mediluxe by the member that remain unpaid for 30 days after its due date for payment.
- The member knowingly provides false details when applying for membership and the false declaration would have reasonably affected Mediluxe's decision to grant the membership.
If Mediluxe cancels the membership for any reason, they reserve the right to retain any monies received to cover any reasonable costs they have incurred as a result.
Other Terms and Conditions:
- Mediluxe reserves the right to vary, add or eliminate any particular services and facilities provided from time to time.
- Mediluxe reserves the right to set aside facilities for social events or activities.
- Mediluxe reserves the right to close or modify facility hours with or without notice.
- May not be combined with any other promotional offer or promotional gift card.
